Investigating
PowerSchool Support is aware this morning (10:30am Eastern, 4 December 2023) of an apparent issue facing multiple PowerSchool Hosted customers.

The Forms link in the Parent Portal is not allowing parents to access Forms and throwing a 502 or possibly 504 network error.
Workaround

As a temporary workaround, customers may wish to disable the SSO connectivity which allows for seamless uploading of documents via the Cloud Document Attachment element.
